研究概要

原発性アルドステロン症は高血圧症の数%を占める二次性高血圧であり、本態性高血圧に比べて心臓や血管、腎臓など様々な臓器に及ぼす悪影響が懸念されることから、的確な診断と治療が求められる。当科は全国でも屈指の原発性アルドステロン症患者受け入れ数を誇り、本疾患の正確かつ効率的な診断や合併症・予後調査に関わる臨床研究を行っている。近年では、segmental AVS(超選択的副腎静脈サンプリング)を用いた副腎部分切除術(副腎皮質温存手術)適応の判断や低侵襲ラジオ波アブレーション治療の治験参加など、最先端の診療を心がけて情報発信している。また基礎研究においては、アルドステロン受容体(ミネラルコルチコイド受容体:MR)に着目し、MRを介した高血圧(MR関連高血圧)発症の分子メカニズムや、腸管でのアルドステロン作用の解析を進めてきた。その結果、高血圧発症における新規MR転写共役因子の関与や、腸管MRによる新規血圧調節機構を明らかにした。現在このような視点に立ちミネラルコルチコイド高血圧の病態解明についてさらなる研究を進めている。

従来の医学書では、アルドステロンによる血圧調節作用は、遠位尿細管~集合管に発現する腎臓のMRが主役と記載されている。本論文では、腸管上皮特異的MR欠損マウスの表現型解析を通して、血圧調節作用において腸管MRが腎MRと同等の寄与であることを明らかにした。このマウスを低塩分食で飼育すると、Na欠乏に陥り、著明な体重減少と血圧低下を認めることから、血圧のホメオスタシスにおいて、腸管MRが極めて重要な意義を持つことが明らかにされた(右図)。
